What Is Osteoarthritis?
Osteoarthritis is a joint disease that affects the whole joint, including cartilage, bone and surrounding joint structures.
Common symptoms include pain during activity, stiffness, swelling, reduced mobility and impaired function. Osteoarthritis most commonly affects the knee, hip, hand, foot and other weight-bearing joints.
Importantly, the severity seen on an X-ray or MRI does not always correspond directly with the amount of pain or functional limitation.

What Causes Osteoarthritis?
Osteoarthritis develops through a combination of biological, mechanical and individual factors.
Previous joint or sports injuries, long-term loading, increasing age, excess body weight, hereditary factors, instability and joint alignment can all influence the development of osteoarthritis.
A specialist assessment helps determine which factors and structures are contributing to your symptoms.
How Is Osteoarthritis Diagnosed?
The diagnosis is based on your symptoms, medical history and an orthopaedic examination together with previous X-rays, MRI or other imaging when relevant.
Musculoskeletal ultrasound can provide additional information when it adds diagnostic value.
The aim is not simply to confirm osteoarthritis, but to understand what is causing your symptoms and which treatment strategy is appropriate.

There is no single treatment that is right for every patient with osteoarthritis.
Treatment depends on the affected joint, extent of the condition, symptoms, function, previous treatment, activity level and individual goals.

PRP Treatment
PRP – Platelet-Rich Plasma – is prepared from the patient’s own blood and is one of the most extensively studied biological injection treatments within orthopaedics, particularly for knee osteoarthritis.
For selected patients, PRP may reduce pain and improve function.

ACP Max
ACP Max is a high-concentration PRP-based treatment that enables a higher platelet concentration and total platelet dose compared with standard ACP.

RFA for Knee Osteoarthritis Pain
For selected patients with persistent osteoarthritis-related knee pain, radiofrequency ablation – RFA – may also be considered.
RFA is not a biological treatment. It is an image-guided minimally invasive method that targets sensory nerve branches around the knee to reduce pain signalling.
RFA does not repair the joint itself but may reduce pain and create better conditions for active rehabilitation.

Advanced Osteoarthritis – Are There Options Before Joint Replacement?
Advanced changes on an X-ray or MRI do not automatically mean that surgery is the only option.
For selected patients, continued non-surgical treatment may still be medically relevant. Depending on the individual situation, treatment may involve rehabilitation, PRP/ACP Max, hyaluronic acid, BMAC or MFAT/Lipogems.
For other patients, joint replacement or another surgical procedure may be the most appropriate treatment.
The goal is not to avoid surgery at all costs. The goal is to determine the right treatment for the right patient at the right time.
Biological Salvage for Advanced Knee Osteoarthritis
Dr Georg Ferm has developed a stepwise Biological Salvage protocol for selected patients with advanced knee osteoarthritis.
The protocol is an individual joint-preservation strategy in which PRP/ACP Max, hyaluronic acid and, for selected patients, BMAC or MFAT/Lipogems may form part of the treatment plan alongside rehabilitation and load management.

Rehabilitation and Surgery
Exercise, strength and appropriate load management are important parts of osteoarthritis treatment and remain relevant even when biological treatment is used.
Biological treatment is not the right option for every patient. With advanced osteoarthritis, significant structural changes or mechanical problems, a surgical assessment may be more appropriate.
